Stop loss trigger never works

I have a simple stop loss trigger.
But whenever it triggers it fails to place the bet in market. I always get the same exact error -- Error changing price ( CANCELLED_NOT_PLACED )
Anyone else face the same issue?
I'll appreciate the help

I have a large offset% percentage because I don't really want to offset the bet but just stop loss it a % loss level
